REPORT OF A SOCCER PLAYER'S DISQUALIFICATION FORM

During the present interscholastic soccer regular season and post-season, a disqualified soccer player (player receiving a red card) shall not be permitted to participate in the next contest in accordance with PIAA's adoption of National Federation's Rule 12-8-3. The head referee must complete and file this form with Bradley R. Cashman, PIAA Executive Director, as soon as possible, but no later than twenty-four (24) hours following the soccer game in which the disqualification took place by forwarding it to the above address. This form is not to be completed for those individuals receiving a SOFT RED card under Rule 12-8-2.
